LIKE LAST YEAR, FORMS MUST BE SUBMITTED TEN (10) WORKING DAYS PRIOR TO PAYMENT DUE DATE.  INTEREST WILL ACCRUE AND MAY EXCEED PAYMENT AMOUNT.  MORTGAGES, HOME EQUITIES AND VISA ACCOUNTS ARE NOT APPLICABLE AND NEW LOANS THAT HAVE NOT HAD A LEAST ONE PAYMENT ARE NOT ELIGIBLE.
